For consideration of a site, please follow the steps outlined below.
1. Print and fill out the following form:
Site Submission Form (pdf)
2. Mail the form and all necessary information to the address below:

3. Please make sure to include the following information when you send your form:
- Site address, including the nearest cross street, county, zip code and state;
- Size of space with site plan
- Annual rent, per sq. ft. rent, taxes, CAM, and insurance
- Zoning designation and municipal requirements;
- Liquor availability
- A vicinity map showing the location of the site and competition (such as convenience/grocery stores, quick serve restaurants, and fast casual restaurants)
- Site photographs and aerial photographs should be included, if available.
- Demographic data (if available)
- Traffic and pedestrian counts (if available)
